Our hours are: Monday thru Friday from 8:30 a.m. to 5:30 p.m.
Additional times available by appointment.

To to make your visits go as smoothly as possible, please read the information below:

Arrive at your first office visit 15-20 minutes to review and complete any paperwork before your appointment.

To save time, complete or update the needed forms prior to your appoint. (see forms below)

Bring your insurance card and a picture ID to each appointment so that your account can be billed correctly.

Early morning and late day appointments at the North Central location available to accomodate working and student patient needs

Spanish speaking and sign language interpretation also provided

What To Wear

Please wear comfortable clothing (t-shirt, shorts or sweats), and exercise shoes.

Payment Procedure and Options

Your copay/deductible is due at the time of your first visit unless other arrangements are made in advance.

We are a preferred provider for most major health plans and we will bill your insurance for therapy services. We accept most insurance company payments, Medicare, Worker's Compensation. We accept most credit cards, checks and cash payments.

If your insurance provider is not listed, please call (210) 490-4738 to see if we can accept your insurance.

It is our policy to bill your insurance company first for your visits. You are, however, responsible for paying any portion not covered by your insurance policy. Since insurance plans change frequently, please call your insurance company to determine if your deductible, limits, or copayment responsibility has changed.
